**Some states do not permit people to use a universal form. So, you cannot use this form if you live in: Indiana, New Hampshire, Ohio, Texas, Wisconsin.  Some states have special requirements for witnesses in certain care facilities. So, you should not use this form if you live in a nursing home or any other care facility in: California, Connecticut, Delaware, New York, Vermont
